PROVIDERS IN REXBURG

Trent Shaw is a country boy who grew up in Utah and Montana, prior to moving to Idaho in 1994. He graduated from Ricks College prior to his stay at Idaho State University where he earned a BS degree in Zoology.

Andrew Flamm was born and raised in Rexburg, Idaho. He graduated from Byu-Idaho with a bachelor’s degree in biology and went on to graduate from ISU’s physician assistant program.

Tate Staker was born and raised in Southeast Idaho. He graduated from BYU-Idaho with a Bachelor’s degree in Exercise Physiology and went on to graduate from Sullivan University's Physician Assistant program in Louisville, Kentucky. After graduating, Tate and his family came back to Idaho.

Shad Crapo was born and raised in Saint Anthony, Idaho. After graduating High School, he attended Brigham Young University-Idaho where he earned a Bachelor of Science Degree in Exercise Physiology. Shad continued his education at Salus University in Philadelphia, Pennsylvania where he earned a Master of Medical Science Degree in Physician Assistant Studies.
